Should you block Mediapartners-Google?

Allow it if
you show Google AdSense or Google Ad Manager ads on this site.
Block it if
you show no Google ads. Remember that User-agent: * does not cover it.
What blocking changes
Google says that if robots.txt disallows the AdSense crawler, it cannot serve Google ads on the site.

To block it, add this group to your robots.txt. Google says Mediapartners-Google ignores the global User-agent: * group. A rule only applies if it names Mediapartners-Google.

User-agent: Mediapartners-Google
Disallow: /

robots.txt only asks. It does nothing against a client that ignores it or only pretends to be Mediapartners-Google. Stopping those takes verification at your edge.
